1. Women With Severe Uterine Abnormalities Who Cannot Carry a Pregnancy
Some women may have congenital absence of the uterus, severe uterine developmental abnormalities, extensive damage to the uterine cavity, or other structural conditions that make carrying a pregnancy extremely difficult or impossible.
Even when ovarian function remains normal and eggs can still be retrieved, the uterus may not be able to provide an appropriate environment for implantation and pregnancy. In such cases, gestational surrogacy may be considered after a comprehensive medical and legal assessment.
2. Women for Whom Pregnancy Would Pose Significant Health Risks
Some women are capable of becoming pregnant but may be medically advised not to carry a pregnancy because of serious cardiovascular conditions, a history of major medical treatment, or other health problems.
In these circumstances, the main purpose of gestational surrogacy is to separate embryo creation from carrying the pregnancy. Embryos may be created using IVF, while the pregnancy is carried by a medically screened gestational carrier.

3. Women With Recurrent Pregnancy Failure Related to Uterine Factors
For some patients who experience repeated pregnancy loss, testing may identify severe intrauterine adhesions, structural uterine abnormalities, or other conditions affecting the pregnancy environment that are difficult to correct.
In these cases, gestational surrogacy may be discussed as an additional option.
However, recurrent miscarriage or implantation failure can also result from embryo chromosomal abnormalities, endocrine conditions, immune-related factors, or other medical problems. If an identifiable and treatable cause is found, addressing the underlying condition should generally come first.
4. Families Who Already Have Frozen Embryos
Some families have already completed egg retrieval, fertilization, and embryo freezing in China or another country, but the intended mother later becomes unable to undergo embryo transfer or safely carry a pregnancy because of changes in her health.
For these families, the main issue may no longer be starting another ovarian stimulation cycle.
Instead, they need to determine whether the existing embryos can legally and medically be transported to the destination country, whether they meet the eligibility requirements for gestational surrogacy, and how embryo transfer, pregnancy management, birth registration, and legal parentage will be handled.
